New Patient Intake Form
As of January 2021 we have adopted a patient portal system. This system allows us to communicate through a health record system , ChARM that can avoid delays and inconveniences of paperwork. Prior to your first appointment, you will receive an invitation to join this portal and be directed to an on-line form to complete.
As a back-up we have kept the New Patient Intake Form here until we are fully functional with the new system.
During transition if you prefer you may use this link to our old form. Save the PDF to your computer, print the form, and then fax or mail to our office. Due to confidentiality we cannot accept email submissions.
